Information About the Survey
Before you take the Religious Survey On-line you need to know a few things:
- No personal information will be used from this survey for solicitation of commercial
goods, services, or fund raising. Personal information is neither published nor provided
on-line. No personal information is furnished for other companies or organizations,
except for local churches for research and follow-up purposes.
- You are entitled to receive the results of the survey periodically via e-mail if you
wish. Check the appropriate box on the form.
- Certain fields are required to verify that the surveys are not redundant. The forms
will not be posted unless these fields are completed. Duplicate entries will be deleted.
- Please be as honest as possible when completing the survey.
